- Monthly salary eligible for coverage = LTD Salary / 12, up to $7,500 per month.
- LTD salary = 140% of total earnings if parsonage is provided; or 110% of total earnings if there is a designated housing allowance; or 100% of total earnings.
- The Spouse Life coverage amount or Dependent Child(ren) Life coverage amount cannot exceed 100% of the Employee Voluntary Life coverage amount, up to a maximum of five times the LTD salary.
- If you have chosen Employee Voluntary Life coverage over $200,000, or Spouse Life coverage over $30,000, you will need to complete an Evidence of Insurability form.
- Employee Voluntary Life coverage is required in order to have Spouse Life coverage and/or Dependent Child(ren) Life coverage.
- Both Employee Voluntary Life and Spouse Life coverage amounts will reduce by 50% when you reach age 70.
